Supplemental Consumer Information Form Required Starting March 1
Effective with applications taken on or after March 1, 2023, the new Supplemental Consumer Information Form must be included with the Uniform Residential Loan Application.
The form requests details concerning the consumer’s participation in a homeownership counseling or housing counseling course. This new form, along with a copy of the consumer’s course completion certificate, must be submitted with the loan file. GMFS underwriters will verify the information, validate that the course meets agency guidelines, and will condition the loan, as necessary.
The form also asks the consumer to specify the language they wish their loan documents to be issued in. At this time, GMFS only issues loan documents and disclosures in English. Any submissions with any other language selected cannot be fullfilled.
